Revert "ship to address" form to previous design

I just noticed today that the "ship to address" form has changed. Now, when editing an address, the titles for the lines like "Name" "Address" "City" "State" are all aligned to the right of the page. 

This is a totally unnecessary change. There's zero benefit, and because we read and write from left to right, it actively makes it harder for us to see which line we're editing.

When you create a new order and fill in the address, the form still populates the line titles to the left, like a regular human being would expect. When editing the address, however, everything changes to right-alignment.
